If you choose to upgrade ChoiceCare with a Zion Health membership, or AMP a DPC or TeleHealth+ enrollment, you are free to choose any hospital or inpatient facility. In fact, Zion will provide financial support to you for expenses incurred overseas, in accordance with their membership guidelines and your membership.


NOTE: For your outpatient services you must choose a provider on the PHCS Specific Services network. Please see the website for more information.
