In-network Co-payment or co-pay is a fixed amount (for example, $15) you pay for covered health care services to providers w ho contract with your health insurance or plan. In-network co-payments usually are less than out-of-network co-payments.

NOTE: For uprgraded ChoiceCare or AMP'ed DPC or TeleHealth+ plans, inpatient or catastrophic medical services shared by Zion, there is no network and you are free to go to any facility of your choice.
